Disability certificate is issued by Health Corporation to those who are disabled due to birth or in any of their lifetime, and depending on the type and percentage of disability. Any disabled person is eligible for the certificate of disability.

Process

In order to apply, the applicant has to complete the form and apply with all the documents mentioned. After this the applicant is screened by the health corporation and the percentage of disability and disability is decided on the basis of pre-determined rules.

Necessary documents

1. Copy of Ration Card
2.  Voter ID  Card
3. Affidavit (If the applicant claims for any insurance)
4. Four photographs
5. Caste Certificate (for a concession of fee)

Fixed charges

1. Rs 32 for general category
2. Rs 8 for Scheduled Castes
